What is CCR ?
It is a connected platform where clinicians need to find out about patients medical history and problems they are facing in an actionable format so they get the gist within 2 minutes so they can direct the patients accordingly.

This research is only based on patients having CKD and clinicians dealing with CKD patients.
What is CKD?

It is usually asymptomatic in the early stages, hence hard to be detected.

No known cure as of now, however treatments can help manage and regulate patient's condition

Given that CKD progression is often irreversible, early interventions are most effective

Once CKD can be diagnosed, glomerular damage has already reached over 50% and is usually irreversible.
Effects of CKD

1. Notification Bar/Page



Simple notification bar to recommend actions to clinicians. Automation was suggested to reduce from 10 touch-points to 1 only. They can chose to work on the suggested recommendation or ignore it. All these tabs are taken care in another product

There are 12-24 different items which shows details of a member medical history. Individually they hold value to nurses and every details changing will be notified here.

2. Timeline




Individual data does not make sense, but when they are combined in the right order with the right time, they can see patterns and information. This generated the idea of a timeline view with all important actions taken in the recent past and will be taken in recent future. This feature will save them time to gather a quick information of the patient.

3&4. Overall Summary & Vital Monitor




We have collected the most important information and vital nurse would like to see to begin with. Overall Summary provides links to details of all the current diseases and Vital monitor provides specific vitals required for a CKD patient. We saved minimum 12 clicks in this feature.

5. Medication Summary



This can quickly display them their medication summary and if they are on the right medication, does it end soon or any reactions they might face. This will act a lifesaver for nurses to find out any possible problems.

6. Gaps in Care


Based on all the other medical data, users will quickly understand the severity of gaps in care, also they can take quick action on it. Three major components- lab, primary condition & doctor’s reports together will tell them if patient is in good condition or might need help.


7. Program Referrals


This shows when and from where the patient was referred from. This details lets user know about the health condition before they became a member with them.
